De quelle partie de l’oreille provient-elle ? Quelle anomalie, quelle pathologie explique votre <strong>hypoacousie</strong> légère, partielle ou sévère ?</p>\r\n \r\n <p style="text-align: justify;">Parlons aujourd’hui des différents types de surdités. Les <strong>oto-rino-laryngologistes</strong> dégagent trois types de surdité bien distincts : la surdité de perception, la surdité de transmission et la perte auditive mixte.</p>\r\n \r\n <h2 style="text-align: justify;">La surdité de transmission</h2>\r\n \r\n <p style="text-align: justify;">Lorsque le médecin parle de <strong>surdité de transmission</strong>, il fait référence à une anomalie située au niveau de l’oreille externe ou de l’oreille moyenne qui empêche une bonne écoute et compréhension des sons. Une obstruction, une anomalie empêche la bonne transmission des sons dans le chemin de l’oreille, d’où l’appellation de surdité de transmission.</p>\r\n \r\n <p style="text-align: justify;">La surdité de transmission est responsable d’une <strong>perte des capacités auditives légères à partielles</strong>. Dans le cas d’une surdité de transmission, les fréquences basses ou graves sont atteintes. Une surdité de transmission n’est jamais à l’origine d’une hypoacousie totale et peut surtout, et c’est une bonne nouvelle pour le patient, se corriger et se soigner.</p>\r\n \r\n <p style="text-align: justify;">Médicaments (cortisone, antibiotique), acte médical ou opération chirurgicale, selon les cas de figure, peuvent corriger la surdité de transmission. Le patient retrouve alors toute sa capacité auditive avec l’aide de son médecin et du traitement adéquat.</p>\r\n \r\n <h2 style="text-align: justify;">La surdité de perception</h2>\r\n \r\n <p style="text-align: justify;">La <strong>surdité de perception</strong>, appelée aussi <strong>surdité neurosensorielle</strong>, est plus grave et problématique que la surdité de transmission. Il s’agit d’un type d’hypoacousie qui touche l’<strong>oreille interne</strong> (et non l’oreille externe ou moyenne) et notamment les terminaisons nerveuses qui transmettent les sons au cerveau.</p>\r\n \r\n <p style="text-align: justify;">La surdité de perception est responsable d’une perte auditive qui peut être légère comme sévère à totale. Dans le cas d’une surdité de perception, les fréquences aiguës sont atteintes en premier, c’est l’inverse pour la surdité de transmission.</p>\r\n \r\n <p style="text-align: justify;">La surdité neurosensielle ou surdité de perception a différentes origines, causes, pathologies et anomalies. Relevons deux sous-catégories de surdités de perception :</p>\r\n \r\n <h3 style="text-align: justify;">Surdité de perception cochléaire</h3>\r\n \r\n <p style="text-align: justify;">Dans ce cas précis, le problème se situe au niveau de la <strong>cochlée</strong>. Qu’est-ce que la <a href="https://mon-centre-auditif.com/blog/cochlee?utm_source=Blog&utm_content=Article-151" target="_blank">cochlée</a> ? L’organe de l’oreille, l’organe de l’ouïe. Avec sa forme d’escargot, cet organe situé dans l’oreille interne est central. Dans la cochlée, on y retrouve l’organe de Corti, un liquide (le périlymphe), mais aussi les cellules ciliées. La cochlée reçoit les vibrations du tympan et les transmet au cerveau sous forme de messages nerveux et électriques<sup>1</sup>.</p>\r\n \r\n <h3 style="text-align: justify;">Surdité de perception rétro-cochléaire</h3>\r\n \r\n <p style="text-align: justify;">Dans cet autre cas, il s’agit d’une anomalie située au niveau du <strong>nerf auditif</strong> et non de la cochlée comme dans le cas précédent. La cochlée réussit à traduire les vibrations en messages nerveux, mais la transmission au cerveau par le nerf auditif ne s’effectue pas correctement. La compréhension du son est donc impossible.</p>\r\n \r\n <h2 style="text-align: justify;">La perte auditive mixte</h2>\r\n \r\n <p style="text-align: justify;">Enfin, troisième cas de figure : la <strong>perte auditive mixte</strong>. Le patient connaît une hypoacousie à la fois de transmission et de perception. Cette situation pour le moins complexe met en évidence des faiblesses et des anomalies au niveau des trois parties de l’oreille à la fois : externe, moyenne et interne.</p>\r\n \r\n <p style="text-align: justify;">Le patient peut connaître une infection chronique aggravée qui ne touche pas seulement l’oreille moyenne, mais aussi l’oreille interne. Typiquement, certains cas d’otospongiose peuvent entraîner une perte auditive mixte. Si à la base cette maladie des os ne touche que les osselets de l’oreille moyenne (cas de surdité de transmission), celle-ci peut s’aggraver et s’étendre à l’oreille interne (cas de surdité de perception). La chirurgie réparatrice (possible en cas de surdité de transmission) n’est alors plus réalisable. La mise en place d’un appareil auditif est alors la seule solution pour aider le patient dans son quotidien.</p>\r\n \r\n <h2 style="text-align: justify;">La surdité de transmission : différents cas de figure</h2>\r\n \r\n <p style="text-align: justify;">L’oreille externe et l’oreille moyenne sont concernées par la surdité de transmission. Conduit auditif externe, tympan ou encore osselets peuvent connaître infections, perforations ou bien d’autres anomalies qui peuvent endommager l’audition.</p>\r\n \r\n <h3 style="text-align: justify;">L’otite aigüe</h3>\r\n \r\n <p style="text-align: justify;">Maladie des enfants, mais pas seulement, cette <a href="https://mon-centre-auditif.com/blog/otite-infection?utm_source=Blog&utm_content=Article-151" target="_blank">infection bénigne de l’oreille</a> typique peut entraîner une perte d’audition éphémère. Otite externe, otite moyenne, otites virales ou otites bactériennes, c’est une infection à prendre au sérieux.</p>\r\n \r\n <h3 style="text-align: justify;">La perforation du tympan</h3>\r\n \r\n <p style="text-align: justify;">Un trou dans le tympan peut entraîner douleur, gêne et perte auditive. Heureusement des antibiotiques, des soins locaux et une hygiène correcte permettent une <strong>cicatrisation de la muqueuse tympanique</strong> rapidement.</p>\r\n \r\n <h3 style="text-align: justify;">Le bouchon de cérumen</h3>\r\n \r\n <p style="text-align: justify;">Fréquent, un mauvais nettoyage des oreilles et du conduit auditif peut entraîner la création d’un <a href="https://mon-centre-auditif.com/blog/bouchon-cerumen?utm_source=Blog&utm_content=Article-151" target="_blank">bouchon de cérumen</a>. Celui-ci bloque les sons. Médecins ou ORL pourront déboucher le conduit et le patient retrouvera son confort d’écoute.</p>\r\n \r\n <h3 style="text-align: justify;">L’ostopongiose</h3>\r\n \r\n <p style="text-align: justify;">Cette maladie de l’oreille moyenne touche plus précisément un os, la <strong>chaîne des osselets</strong>. Chez une personne en bonne santé, les osselets qui ont pour vocation de transformer les ondes sonores en vibrations et de les transmettre à l’oreille interne, se renouvellent. <br />\r\n Les osselets sont en effet, des os qui se renouvellent tout au long de la vie. L’<a href="https://mon-centre-auditif.com/blog/otospongiose?utm_source=Blog&utm_content=Article-151" target="_blank">otospongiose</a> empêche la régénération des osselets. Ainsi ces petits os s’usent et ne parviennent plus assez bien à remplir leur mission. Le chirurgien ORL propose une intervention : retirer l’os atteint et le remplacer par une prothèse. Le patient retrouve alors une audition quasi normale.</p>\r\n \r\n <h2 style="text-align: justify;">La surdité de perception : différents cas de figure</h2>\r\n \r\n <p style="text-align: justify;">Quelles sont les pathologies et autres <strong>troubles de l’audition</strong> qui peuvent toucher directement l’oreille interne et être à l’origine d’une surdité de perception ?</p>\r\n \r\n <h3 style="text-align: justify;">La presbyacousie</h3>\r\n \r\n <p style="text-align: justify;">Nous en avons déjà parlé, ici, la <a href="https://mon-centre-auditif.com/blog/presbyacousie?utm_source=Blog&utm_content=Article-151" target="_blank">presbyacousie</a> ou le vieillissement naturel des oreilles est l’une des principales hypoacousies connues. C’est une surdité de perception, car les <strong>cellules ciliées</strong> dans l’organe de Corti se meurent, tout simplement.</p>\r\n \r\n <h3 style="text-align: justify;">Les médicaments ototoxiques</h3>\r\n \r\n <p style="text-align: justify;">Certains médicaments se révèlent toxiques pour l’oreille interne, ils sont <a href="https://mon-centre-auditif.com/blog/tout-savoir-sur-l-ototoxicite?utm_source=Blog&utm_content=Article-151" target="_blank">ototoxiques</a>. Leur prise en continu peut entraîner des lésions graves de l’oreille interne.</p>\r\n \r\n <h3 style="text-align: justify;">Les nuisances sonores et la pollution sonore</h3>\r\n \r\n <p style="text-align: justify;">L’<a href="https://mon-centre-auditif.com/blog/pollution-sonore?utm_source=Blog&utm_content=Article-151" target="_blank">excès de bruit</a> au quotidien sur-stimule notre ouïe et l’abîme profondément. Les cellules ciliées comme les nerfs se retrouvent saturés et traumatisés. Alors, une seule chose, baissez le volume !</p>\r\n \r\n <h3 style="text-align: justify;">Le tabagisme</h3>\r\n \r\n <p style="text-align: justify;">Le tabagisme actif comme passif entraîne une perte auditive. Cela a été prouvé scientifiquement.
