Il s’agit des produits, médicaments ou substances qui, par leur composition, sont toxiques pour les oreilles, l’audition et les cellules auditives. </p>\r\n \r\n <p style="text-align: justify;"> </p>\r\n \r\n <h2 style="text-align: justify;">Perte auditive et médicaments ototoxiques</h2>\r\n \r\n <p style="text-align: justify;">Sur ce blog, nous avons, à plusieurs reprises, discuter des diverses causes de la <strong>perte auditive</strong> : la <a href="https://mon-centre-auditif.com/blog/age-facteur-determinant-perte-audition?utm_source=Blog&utm_content=Article-82">presbyacousie</a>, les chocs acoustiques, la génétique, les <a href="https://mon-centre-auditif.com/blog/otite-infection">infections virales</a>, bactériennes mais aussi la génétique. Bref, pour de multiples raisons, nous pouvons connaître dans notre vie une <strong>baisse de son audition</strong> et une baisse de notre capacité auditive. La mise en place d’aides auditives et d’audioprothèses est alors une solution pour palier cette perte et mieux vivre son quotidien.</p>\r\n \r\n <p style="text-align: justify;">Nous n’avons, en revanche, jamais abordé encore l’ototoxicité ou la toxicité de certains médicaments pour l’oreille interne.</p>\r\n \r\n <p style="text-align: justify;">En effet, la prise de certains médicaments précis peut avoir des répercussions et des conséquences directe sur notre ouïe.</p>\r\n \r\n <p style="text-align: justify;">Attention cependant, si de nombreuses substances chimiques sont ototoxiques et se retrouvent dans des médicaments que nous consommons, certains facteurs extérieurs sont à prendre en compte pour comprendre une baisse d’audition :</p>\r\n \r\n <ul>\r\n \t<li style="text-align: justify;">l’âge de la personne,</li>\r\n \t<li style="text-align: justify;">le terrain génétique défavorable,</li>\r\n \t<li style="text-align: justify;">une personne déjà sous traitement et vulnérable,</li>\r\n \t<li style="text-align: justify;">mais aussi et surtout la posologie, la durée du traitement, la manière dont est pris ou administré le médicament.</li>\r\n </ul>\r\n \r\n <p style="text-align: justify;"> </p>\r\n \r\n <h2 style="text-align: justify;">Les fameux effets secondaires</h2>\r\n \r\n <p style="text-align: justify;">L’ototoxicité est signalée sur les notices des médicaments bien souvent dans la partie effets secondaires. Selon leurs substances et leurs familles, ils peuvent être toxiques :</p>\r\n \r\n <ul>\r\n \t<li style="text-align: justify;">pour la <strong>cochlée</strong>, on parlera de toxicité cochléaire ou cochléotoxique,</li>\r\n \t<li style="text-align: justify;">pour le <strong>vestibule</strong>, une autre partie de l’oreille interne, on parlera alors de toxicité vestibulaire. Dans ce cas le patient pourra ressentir des vertiges et un déséquilibre.</li>\r\n </ul>\r\n \r\n <p style="text-align: justify;">L’ototoxicité médicamenteuse peut donc toucher diverses parties de l’oreille selon la molécule du médicament : les cellules ciliées externes, les cellules ciliées internes mais aussi l’<strong>organe de l’équilibre</strong>.</p>\r\n \r\n <p style="text-align: justify;"> </p>\r\n \r\n <h2 style="text-align: justify;">Quels sont les risques des médicaments ototoxiques ?</h2>\r\n \r\n <p style="text-align: justify;">Selon le médicament en question, la prescription et la durée du traitement, un médicament ototoxique qui va venir perturber le bon fonctionnement de l’oreille interne peut produire chez le patient :</p>\r\n \r\n <ul>\r\n \t<li style="text-align: justify;">des vertiges,</li>\r\n \t<li style="text-align: justify;">des troubles de l’équilibre,</li>\r\n \t<li style="text-align: justify;">des <strong>acouphènes</strong>,</li>\r\n \t<li style="text-align: justify;">mais aussi et surtout une altération de l’audition. Une perte de l’<strong>audition des aigües </strong>est souvent la plus répandue.</li>\r\n </ul>\r\n \r\n <p style="text-align: justify;">Car oui, il ne faut pas l’oublier, l’oreille interne est autant utile pour transmettre le message auditif au cerveau que pour notre équilibre général dans l’espace.</p>\r\n \r\n <p style="text-align: justify;"> </p>\r\n \r\n <h2 style="text-align: justify;">Quels médicaments sont ototoxiques ?</h2>\r\n \r\n <p style="text-align: justify;">Dans la catégorie des médicaments dits ototoxiques, nous retrouvons essentiellement :</p>\r\n \r\n <ul>\r\n \t<li style="text-align: justify;">certains antibiotiques aminoglycosides,</li>\r\n \t<li style="text-align: justify;">certains antiobiotiques macrolides ,</li>\r\n \t<li style="text-align: justify;">certains médicaments utilisés en cas de chimiothérapie à base de sels de platine,</li>\r\n \t<li style="text-align: justify;">certains médicaments antipaludiques,</li>\r\n \t<li style="text-align: justify;">certains médicaments de la famille des anti-inflammatoires : certains salicylates (aspirine) et certains non-stéroïdiens avec la molécule ibuprofène et la molécule Naproxen.</li>\r\n \t<li style="text-align: justify;">ou encore certains antidépresseurs et certains antiarythmiques.</li>\r\n </ul>\r\n \r\n <p style="text-align: justify;">La liste des médicaments ototoxiques est plutôt longues. Les médecins estiment qu’ils seraient une centaine à pouvoir endommager les capacités auditives.</p>\r\n \r\n <p style="text-align: justify;">Certains médicaments sont reconnus ototoxiques tandis que d’autres le deviennent après un traitement long ou un dosage trop important.</p>\r\n \r\n <p style="text-align: justify;"> </p>\r\n \r\n <h2 style="text-align: justify;">Dans quels cas ce type de médicaments sont-ils prescrits ?</h2>\r\n \r\n <p style="text-align: justify;">Les <strong>antibiotiques aminoglycosides</strong> peuvent être prescrits dans le cadre d’un traitement pour soigner une turbeculose multi-résistante ou en cas d’infections graves. Quant aux <strong>antibiotiques macrolides</strong>, ils sont prescrits pour des infections ORL mais aussi pulmonaires.</p>\r\n \r\n <p style="text-align: justify;">Dans le cadre d’une <strong>chimiothérapie</strong>, la cisplatine et le carboplatine sont les substances les plus à craindre.</p>\r\n \r\n <h2 style="text-align: justify;"><br />\r\n Quelles solutions ?</h2>\r\n \r\n <p style="text-align: justify;">Les chiffres publiés par <a href="https://doi.org/10.1016/S2214-109X(23)00514-4">Lancet Global Health</a>, une revue médicale reconnue, sont effarants. De très nombreuses personnes sont, chaque année, exposées dans le monde à des médicaments et des traitements toxiques pour leurs oreilles et leur audition.</p>\r\n \r\n <p style="text-align: justify;">Les chercheurs estiment que plus de 33 millions de personnes dans le monde connaitraient une <strong>baisse de l’audition</strong> ou une perte auditive suite à un traitement incluant un médicament ototoxique.</p>\r\n \r\n <p style="text-align: justify;">Une meilleure connaissance de la part du corps médical est une première proposition pour contrer le phénomène. Les médecins doivent être plus sur le qui-vive et doivent proposer d’autres médicaments quand le cas le permet. Des alternatives existent. Il suffit de les connaître. Pour cela il faut se documenter et mener une veille constante. Il faut réadapter les traitements et les posologies.</p>\r\n \r\n <p style="text-align: justify;">Une sensibilisation du public comme des médecins aux risques de ces médicaments qui manquent encore d’être bien connus et identifiés est une deuxième piste.</p>\r\n \r\n <p style="text-align: justify;">Aussi, la recherche scientifique et médicale doit travailler à développer activement des <strong>agents otoprotecteurs</strong>.
