Les vertiges peuvent être dûs : </p>\r\n \r\n <ul>\r\n \t<li style="text-align: justify;">à un problème cardiaque,</li>\r\n \t<li style="text-align: justify;">à un problème neurologique,</li>\r\n \t<li style="text-align: justify;">ou encore liés à l’oreille interne.</li>\r\n </ul>\r\n \r\n <p style="text-align: justify;">Nous nous intéresserons ici aux <strong>vertiges liés à l’oreille interne</strong>, ils sont assez fréquents. <br />\r\n </p>\r\n \r\n <h2 style="text-align: justify;">Les cristaux, l’oreille et l’équilibre</h2>\r\n \r\n <p style="text-align: justify;">Si cela semble pour le moins étonnant, l’oreille ne sert pas seulement à entendre. Elle possède un rôle majeur dans notre <strong>équilibre</strong>. L’oreille interne, plus exactement, est un organe essentiel à notre sens de l’équilibre.</p>\r\n \r\n <p style="text-align: justify;">Pour mieux comprendre ce rôle, il faut connaître les différentes parties de l’oreille interne. Celle-ci est composée de la cochlée, du vestibule et de canaux semi-circulaires. Dans ces canaux plein de liquide flottent des <strong>otolithes </strong>(appelés familièrement cristaux), il s’agit de cailloux de calcium microscopiques. Lorsque la tête bouge, les otolithes se déplacent. Le tout reste à l’horizontale, quelque soit l’inclinaison de la tête.</p>\r\n \r\n <p style="text-align: justify;">Pour de multiples raisons (infections virales ou bactériennes, génétique, physique) cet équilibre dans l’oreille interne peut se voir troublé.</p>\r\n \r\n <p style="text-align: justify;"> </p>\r\n \r\n <h2 style="text-align: justify;">Comment reconnaître les vertiges liées à l’oreille interne ?</h2>\r\n \r\n <p style="text-align: justify;">Plusieurs signes peuvent vous aider à reconnaître des vertiges provoquées par l’oreille interne :</p>\r\n \r\n <ul>\r\n \t<li style="text-align: justify;">une sensation de rotation désagréable de votre environnement après avoir bouger la tête, il s’agit alors de <strong>vertiges positionnels</strong> (cas typique de VPPB, voir plus bas dans l’article),</li>\r\n \t<li style="text-align: justify;">un mouvement involontaire des yeux appelé <strong>nystagmus</strong>,</li>\r\n \t<li style="text-align: justify;">des problèmes auditifs, troubles ou <strong>perte auditive.</strong></li>\r\n </ul>\r\n \r\n <p style="text-align: justify;">Si vous avez connu des infections de l’oreille par le passé, des troubles auditifs, vous êtes aussi plus enclin à développer des vertiges.</p>\r\n \r\n <p style="text-align: justify;">Si vous souffrez de vertiges dus à l’oreille interne, vous pourrez aussi constater une perte d’équilibre, de la fatigue, un état nauséeux, des problèmes d’audition mais aussi des bouffées de chaleur.</p>\r\n \r\n <p style="text-align: justify;"> </p>\r\n \r\n <h2 style="text-align: justify;">Que faire si les vertiges sont liés à un problème d’oreille interne ?</h2>\r\n \r\n <p style="text-align: justify;">Consulter votre médecin généraliste est déjà indispensable. Ce dernier pourra vous orienter vers un ORL, <strong>Oto-Rhino-Laryngologue</strong>. Ce médecin spécialiste de l’oreille, du nez et de la gorge saura établir un bilan approfondi de l’oreille interne et vous soigner. Il saura surtout après observation poser un diagnostic sur la cause exacte de vos vertiges.</p>\r\n \r\n <p style="text-align: justify;">Cette étape est obligatoire pour respecter le parcours de soins coordonnés, et ainsi être mieux remboursé sur les consultations avec un spécialiste ou les soins qui seront prescrits (voir le <a data-sk="tooltip_parent" data-stringify-link="https://goodassur.com/mutuelle-sante/remboursement/medecin-traitant" delay="150" href="https://goodassur.com/mutuelle-sante/remboursement/medecin-traitant" rel="noopener" target="_blank">simulateur de remboursements de Goodassur</a>).</p>\r\n \r\n <h2 style="text-align: justify;">Les vertiges liés à l’oreille, plusieurs pathologies possibles</h2>\r\n \r\n <p style="text-align: justify;">Deux pathologies de l’oreille se distinguent particulièrement dans le cas des vertiges :</p>\r\n \r\n <ul>\r\n \t<li style="text-align: justify;">La pathologie la plus commune, mais aussi la moins grave, est le VPPB, le <strong>vertige paroxystique positionnel bénin</strong>.</li>\r\n \t<li style="text-align: justify;">L’autre pathologie de l’oreille qui provoque des vertiges assez répandue est la <strong>névrite vestibulaire</strong>.</li>\r\n </ul>\r\n \r\n <p style="text-align: justify;">Dans le cas du VPPB, les vertiges sont plutôt brefs. Il s’agit de <strong>vertiges temporaires</strong> qui se manifestent lors de certains mouvements de tête. L’environnement tangue et la sensation est désagréable.</p>\r\n \r\n <p style="text-align: justify;">Dans le cas de la névrite vestibulaire, les vertiges sont plus longs, ils sont continus. Des nausées, et même des vomissements, peuvent survenir. La <strong>sensation de déséquilibre et d’instabilité</strong> est lourde et très pénible à supporter.</p>\r\n \r\n <p style="text-align: justify;">D’autres pathologies auditives peuvent occasionner des vertiges :</p>\r\n \r\n <ul>\r\n \t<li style="text-align: justify;">Les <strong>infections du labyrinthe</strong>, d’origine virale, peuvent causer, mais pas toujours, des vertiges sévères.</li>\r\n \t<li style="text-align: justify;">La <strong>maladie de Ménière</strong> provoque une perte d’audition, des acouphènes mais aussi des crises de vertiges intenses, des crises de vertiges rotatoires.</li>\r\n \t<li style="text-align: justify;">Le <strong>neurinome de l’Acoustique</strong> est une tumeur bénigne qui provoque des troubles de l’audition mais aussi des vertiges.</li>\r\n </ul>\r\n \r\n <p style="text-align: justify;"> </p>\r\n \r\n <h2 style="text-align: justify;">Focus sur le VPPB</h2>\r\n \r\n <p style="text-align: justify;">Dans le cas d’un VPPB, les <strong>cristaux de l’oreille interne</strong> sont troublés et lancent des messages faussés au cerveau. Un VPPB peut se déclencher suite à un mouvement de tête ou une rotation trop brusque vers l’arrière ou vers le bas. Les tous petits cristaux de calcium dans les canaux de l’oreille interne sont soudain troublés et se déplacent. Ils ne parviennent plus à faire leur travail : envoyer les bonnes informations au cerveau, ils sont déboussolés car ils ont bougé. Leur mauvaise transmission provoque des vertiges.</p>\r\n \r\n <p style="text-align: justify;">Assez courant, le VPPB peut être pris en charge par le médecin généraliste. Ce dernier peut prescrire des médicaments antiémétiques et des vestibulolytiques. Si les manifestations de déséquilibre persistent alors des séances chez un kiné spécialisé peuvent être prescrites. Le kiné saura rétablir l’équilibre des cristaux avec des mouvements et des gestes bien spécifiques de la nuque et de la tête (<strong>manœuvre d’Epley</strong>). Il replace les cristaux dans une partie de l’oreille où ces derniers ne provoquent pas de symptômes de vertiges. Plusieurs séances sont souvent nécessaires afin de retrouver un bon équilibre et ne plus avoir cette sensation désagréable d’environnement qui tangue, qui tourne !</p>\r\n \r\n <p style="text-align: justify;"> </p>\r\n \r\n <h3 style="text-align: justify;">La neuronite vestibulaire</h3>\r\n \r\n <p style="text-align: justify;">Particulièrement intense et douloureuse, la névrite se soigne bien et disparait après plusieurs semaines ou mois. Il s’agit simplement d’une <strong>inflammation du nerf vestibulaire</strong> qui empêche la bonne transmission de messages au cerveau.
